We collaborate with partners to improve the end of life and emotional wellbeing care and support available to the patients and clients of today and future generations.
We are keen to work closely with partners in the NHS, local authorities and other private and voluntary care providers involved with our patients and the general provision of care and support services.
We want to work in collaboration with such health and social care partners so that every person gets the personalised care they need. Our aim is to offer services that complement the help and support already provided by other agencies.
We are also working hard to influence the planning and development of end of life care and emotional wellbeing support services across the areas we serve. Much of this work is focused on achieving seamless delivery of services between agencies, finding joint working opportunities that ensure local people’s needs are met, developing strategies to cope with the increased need, and securing the monies needed to fund these initiatives.
