Almost all of our services are accessed via a referral from a healthcare professional involved in your care. Our list of services below outlines referral information and forms. Please only use these forms if you are referring a patient to Saint Michael’s.
At Saint Michael’s we know that when people are coming to the end of their life, they wish to receive care and support in the place they call home, in familiar surroundings with family, friends and the things that matter to them.
Our expert team of healthcare assistants visit people in their own homes to provide care in various ways. This could be personal care, support with toileting, washing, mouthcare, dressing, positional changes and bedding changes, as well as supporting the patient and their family with their emotional wellbeing.
Referrals to this service must be made by a professional, and this is done over the phone by calling our colleagues in the Harrogate End of Life Coordination Team on (01423) 553 464, Monday-Friday, 8.30am-4.30pm.
Saint Michael’s offers specialist help and support to individuals who are affected by this condition, and is the only provider of this specialist service in the Harrogate district.

Saint Michael’s Inpatient Unit (IPU) offers expert, 24-hour care at the end of life, as well as 24-hour symptom management.

Saint Michael’s Wellbeing Network offers physical, practical, emotional, social and spiritual support, tailored to a person’s individual needs.
Together with health and social care partners across the region, Saint Michael’s is developing a range of outpatient services to help local people access the right support, at the right time, in the right place. The support available includes outpatient clinics to help people affected by terminal illness to manage symptoms, as well as accessing peer support groups and professional advice.
Referrals can be made for patients, families, and carers over the age of 18, affected by terminal illness.

Saint Michael’s Advance Care Planning (ACP) Service supports people living with any palliative diagnosis across the Harrogate district to make choices about their future care, ensuring care choices at the end of their lives are discussed and noted, so that they may be cared for with their wishes in mind.
The service is available to anyone across the Harrogate district with any palliative diagnosis, with support offered in a variety of places, including hospital settings, care homes, Saint Michael’s Hospice and at home.
The service is funded by Harrogate and District NHS Foundation Trust and Macmillan Cancer Support, in partnership with Social Finance. It forms part of the Harrogate End of Life Planning and Support Service (HELPSS).

Breathing Space is a weekly outpatient clinic for people living with a terminal illness and experiencing breathlessness. The session offers practical exercises, advice and support to manage symptoms and improve quality of life.

Just ‘B’ is part of Saint Michael’s family of services and provides bereavement and emotional wellbeing support to children, young people and adults across North Yorkshire. Just ‘B’ is accessed by both self-referral and professional referral. Find out more on the Just ‘B’ website.
From time to time we also receive a small number of referrals for our services from outside our catchment area. In these cases, if the patient is medically suitable and our services are available, Saint Michael’s staff will make an out of area request to the appropriate commissioner for funding. If this is granted, we will accept the patient for care.
