Referrals and accessing our services

Our care is available to people living in the Harrogate district. All our services are free for patients and their families or carers. We provide care for people aged 18-years-old and upwards.

If you think you could benefit from using a Saint Michael’s service, please talk to your GP, district nurse, specialist nurse, consultant or any other healthcare professional involved in your care about your options.

How to make a referral

If you are a health or social care professional wishing to refer someone to Saint Michael's please download the referral form below.

Please note that referrals to all our clinical services are by professionals only.

Alternatively all referrals can be made electronically via SystmOne.

If you wish to know if an urgent referral has been received please call 01423 879 687 – opt 1 and then opt 3.

For all other enquires about referrals please call 01423 879 687 – opt 1 and then opt 2.

Referrals outside the Harrogate district

From time to time we also receive a small number of referrals for our services from outside our catchment area. In these cases, if the patient is medically suitable and our services are available, Saint Michael's staff will make an out of area request to the appropriate commissioner for funding. If this is granted, we will accept the patient for care.
